Milton is telling Kevin that his clean cut, powerful lawyer look, is responsible for his wife's death. While Mary Anne was miserable in New York, Kevin refused to go back to Florida and loose his position in the firm. I agree that Kevin's vanity is responsible for his wife's death, and I feel that the ending was completely appropriate.
